Children aged 3 and 17 years who have been diagnosed with a life threatening condition and are resident in the UK are eligible to make a magical wish!
A wish gives the chance to do something extraordinary. For many children, a wish is an opportunity to do something they never thought possible when life is tough. Children's wishes can range from being a princess for the day or meeting their hero, to going on a once-in-a-lifetime family holiday.
To have their wish granted, your child needs to be a UK resident, aged between 3 and 17 years, and fighting a life-threatening condition such as Vascular EDS.
The make a wish team will speak to your child's consultant to confirm their Vascular EDS diagnosis.
In addition, your child should not have received a previous wish from them, or from another wishgranting organisation – they are only able to grant one wish to a child.
They can accept a wish application if they receive it on your child's third birthday, or before the day of their 18th birthday.
Download the short form that you'll need to print off and post back to Make-A-Wish.
Please remember that the child's parent/guardian needs to sign the medical release form – this will let allow them to write to the child's consultant as soon as possible, in order to establish medical eligibility. Please post the form back to: Make-A-Wish Foundation UK, 329-331 London Road, Camberley, Surrey, GU15 3HQ.
If you have any problems downloading or completing the referral form, please contact Make-A-Wish on 01276 40 50 70 or email firstname.lastname@example.org and they will send you a form in the post.
Once Make-A-Wish have spoken to your child's medical consultant and they've confirmed they have Vascular EDS, they will send a short form asking for further information about your nominated child.
When they receive the form back, they will arrange for specially trained volunteers to visit the child to find out what their wish is.
Good Luck & Best Wishes !
